Terms Of Service
CONFIDENTIALITY
There are legal and ethical provisions that protect your confidentiality in all but a few circumstances. The circumstances in which we cannot guarantee confidentiality legally and/or ethically are: 1.) when we believe you intend to harm yourself or another person, 2.) when we believe a child, elder, or disabled person has been or will be abused or neglected, or 3.) when ordered by a judge to release information. Outside of these circumstances we are bound to protect your confidentiality privilege.
If you would like us to share information with another party, you may sign a release of information. This form states exactly what information is to be shared, with whom, and why, and it also sets time limits.

FEES AND PAYMENT
Our office-based services rate is $300 per hour. Our house call rate is $395 for the first hour and $300 per subsequent hours, including time in your home and travel time, billed in half hour increments. You may also be charged for additional time spent on your care. This may include time spent interacting with pharmacies, other physicians and medical facilities as indicated.
If you live outside of Asheville, you may be charged an additional travel fee, depending on distance. If so, this will be discussed with you during your initial phone contact.
If your treatment will require use of higher cost disposable equipment, you may be charged the at-cost replacement fee for these items. Routine disposable equipment and supplies are provided at no cost.
We will provide an estimate of all costs for your visit during our initial phone conversation. We accept credit card and cash payment, including payment through your Health Savings Account (HSA). Your card information may be collected when you book your appointment, however we will not charge your card until your visit is completed and we have reviewed your invoice with you. Payment is made in full at the time of the visit. Travel Fee may be collected up front for clients outside Asheville.
Phone and videoconferencing consults are available for prescription refills or other routine needs, if we determine this is medically appropriate. These are billed in 15-minute increments with a typical rate of $90 for a 15-minute call and $150 for a 30-minute call.
PRIVATE PAY NATURE OF PRACTICE
Payment is due in full at the time of service. We do not participate in insurance company networks and do not directly bill private insurance. If you prefer, we will fill out a billing invoice, which is provided to you only at your request, to allow you to file with your insurance directly and request partial reimbursement for services provided.
EMERGENCIES
In the event of a life-threatening emergency, PLEASE CALL 911 and inform them you have a LIFE-THREATENING EMERGENCY. As soon after this as possible, please contact us (or authorize someone else to contact us) so that we can provide any service that you, or your care providers, might require. For concerns that are not urgent, please call to request an appointment.
IF WE NEED TO CONTACT SOMEONE ABOUT YOU
If there is an emergency during your treatment, or if we become concerned about your personal safety, we are required by law and by the rules of our profession to contact someone close to you, such as a relative, spouse, or close friend. We are also required to contact this person, or the authorities, if we become concerned about your harming someone else. Please provide us with an emergency contact where prompted on your intake paperwork.
